We are requesting your participation in one of the two surveys below to learn about your experiences as a worker during a pandemic.
During a health crisis of this nature, access to objective information on the experiences of frontline workers is vital in helping support the best possible solutions. Led by the Occupational Health Clinics for Ontario Workers, with researchers from Institute of Work & Health and Duke University in North Carolina and the input of many unions and other labour friendly organizations, a survey for the experience of health care workers was developed.
Health Care Worker Survey:

F - https://www.surveymonkey.com/r/sondage_pandemie
The tremendous success of the first survey resulted in a second survey for all workers:
Non-Health Care Worker Survey:

F - https://www.surveymonkey.com/r/pandemie_sondage
During the pilot study, about half of the respondents took less than 20 minutes to complete the survey (the shortest time was 10 minutes). Those who went through the survey slowly and provided thoughtful comments took up to half an hour.
If you feel uncomfortable answering a question, skip it. All the information collected in this survey is confidential. Only the researchers directly involved in this project will see all the answers. The team will not keep any information that could identify you or others. All the data collected will be stored on a secure server.
